ST AUSTELL HEALTHCARE’s Privacy Policy applies to this competition. You can read more about it here.

If entrants are under 16, their parent or guardian must consent to their entry and indicate this consent on the entry form to confirm that the entrant has complied with these terms and conditions. Proof of age, identity and eligibility may be requested.

Entrants must submit their ideas and designs to support an app that will link children and young people to local resources, opportunities and support around St Austell, Mevagissey, Gorran Haven and Foxhole.

The competition launches on 30 June 2021.

Entries must be submitted via the uploader on the competition website https://dreamquest.uk/! The maximum file size that the uploader can accept is 10 megabytes.

The uploader can accept the following file types: .pdf.

Entrants must also complete the online entry form as part of the upload process. St Austell Healthcare will only use personal details for the purposes of administering this competition. Please see the St Austell Healthcare’s Privacy Policy.

Closing date for entries is July 31st 2021. Submissions received after this time will not be considered, so please do not wait until the last minute to upload your entry. Entries will not be returned so please remember to keep a copy.

Only one entry per individual is permitted.

Entries must be completely original and entirely the entrants own work.

Branded products should not feature in the entry.

Entries must not contain defamatory, obscene, offensive or any other unsuitable material. Entries must be suitable to be applied by St Austell Healthcare to audiences of all ages.

If the entry features any people other than the entrant, permission should be obtained from all involved for the entry to be submitted in the competition (or their parents/guardians if under 16).

St Austell Healthcare accepts no liability if entrants ignore these rules, and entrants agree to indemnify fully St Austell Healthcare against any claims by any third party arising from any breach of these rules.

Entrants retain the copyright in their entry but grant to Patientcards Ltd a non-exclusive licence to use the entry across our media including any online platforms and use on https://www.staustellhealthcare.co.uk/ .

Entrants permit St Austell Healthcare /Patientcards Ltd to edit or change entries in any way it may in its sole discretion that it may consider necessary for the purposes of use in the competition.

By entering the competition, entrants warrant that they have legal capacity to enter the competition and agree to be bound by these terms and conditions.

Individual entrants under 16 must seek approval from a parent or guardian as a condition of entry. If, after making reasonable attempts, St Austell Healthcare cannot confirm with the parent or guardian that consent has been granted the entry will be disqualified.

If the winner or runners up cannot be contacted after reasonable attempts have been made to do so, St Austell Healthcare reserves the right to offer the prize to the next best entry.

The overall winner will be awarded a Dream Quest prize (up to the value of £250), of their choice.

The winner grants all necessary rights and licences to enable Patientcards Ltd to create an app.

The two runners up will receive a voucher to the value of £150 towards an activity of their choice.

Entrants must agree to take part in any post-competition publicity if required.

St Austell Healthcare’s decision is final and no correspondence will be entered into. Unsuccessful entrants will not be contacted.

St Austell Healthcare reserves the right to disqualify any entry which breaches any of these Terms and Conditions or to withhold or withdraw a prize if in its opinion entries do not reach the required standard.

St Austell Healthcare reserves the right to cancel this competition at any stage, if deemed necessary in its opinion, and if circumstances arise outside its control. In this event, a notice will be posted on the St Austell Healthcare website: https://www.staustellhealthcare.co.uk/.

St Austell Healthcare, its sub-contractors, subsidiaries and/or agencies cannot accept any responsibility whatsoever for any technical failure or malfunction or any other problem with any server, Internet access, system or otherwise which may result in any entry being lost or not properly registered or recorded. Proof of sending is not proof of receipt.

These Terms and Conditions are governed by the laws of England and Wales.

Entries can also be posted in person at your local St Austell Healthcare Surgery, at either one of the 3 sites in the town, or at the Mevagissey or Foxhole surgeries.
